Complete your patient form on the patient portal

You can complete the patient forms your practitioner sends you directly through your patient portal. Filling in your forms ahead of time helps your practitioner prepare for your appointment and ensures your information is recorded accurately. You can access and submit forms from any device, making the process simple and convent.

Submit patient forms from your patient portal

  1. On the side menu, click Records > My Forms.

  2. In the list, next to the form that you need to fill out, click Submit Form.

  3. Complete the form as required, then click Submit.

When you've submitted the form, a View Form button appears, which allows you to view your answers.

Note

For forms that require co-signing with the practitioner, the View Form button only appears when the practitioner has co-signed the form.
