Your Halaxy health notes are the parts of your medical record that your practitioners have chosen to share with you. Essentially, they are your clinical history.
Health notes may be treatment notes, referrals, prescriptions, allergies, diagnoses, immunisations, clinical questionnaires or even annotatable charts and images.
You can also create your own notes and upload files to share with your practitioner.
Any clinical forms or questionnaires your practitioner wants you to fill out online will appear as health notes.
To view all the health notes that are shared with you and created by you, navigate to My Health > Notes.
In the Notes page, you can find the following:

Appointments & Shared Notes list: The list of all your appointments and clinical notes that are shared with you. This can be past appointments, shared clinical notes, prescriptions, referrals, questionnaires, allergies, diagnoses, immunisations, and other relevant shared records. Items on the list can be selected to open their details.

My Notes list: The list of your own created notes. Items on the list can be selected to open their details.

List filters and settings: A handy set of tools and settings to filter out the items in the lists.

Search field used to search for notes using keywords.

Filters to select the note category, date range and authors to show in the lists.

Sorting options to sort the list chronologically or alphabetically.

Settings to choose to show an expanded or condensed view of the lists.

Note title: The title of the note that is selected.

Note/File information: Key information that is relevant to the clinical note. This includes the linked appointment details (if any), the sharer of the note and any to whom the note is shared.

Note content: The message and contents of the selected note itself.

Actions menu: A button with a dropdown menu featuring a list of actions you can do with the selected clinical note.

Quick action toolbar: A readily available selection of actions you can do in the notes page.
You can create your own note for your own use or to share with a practitioner.

Navigate to My Health > Notes.

In the quick actions toolbar located in the top-right, click on New Health Note.

Click the
pencil icon next to the note title to update the title, date or to link the note to a specific appointment.

Click Add File to upload an attachment to the note.

Compose the content of the note in the note content section. You can use the toolbar at the top of the text field to properly format your content.

On the top-right, click Save to save the changes to the note. Additionally, in the dropdown next to it, you can Publish, Share, Preview, Print or Delete the note.
Note: Your saved clinical notes can be found under the My Notes list.

Navigate to My Health > Notes.

In the quick actions toolbar located in the top-right, click on Upload a File.

Drop a file in the upload box or click Upload File to manually locate and select a file.

Give the file a title, comments (optional), and select a category.

Click Save. The file will be saved under the My Notes list.
Sometimes your practitioner may ask you to create your own notes or share files with them.

Share notes

Select a note from the list on the left.

On the top-right, click the dropdown next to Save (or Revert), and select Share.

Select a practitioner you want to share with in the dropdown and click Save.
The practitioner will be notified that a note has been shared with them, which they can access in your patient profile on their end.

Share files

Select a file from the list on the left.

Under the file information, click on Share.

In the pop-up, under Share with, select if you want to share to a User (this can be a family member with a Halaxy account), or Practitioner.

In the dropdown under User or Practitioner, select whom you want to share the file with.

Click Save.
If the file is shared with a user, they will be able to access the file when they log in to their own account. For practitioners, they will be notified that you have shared a note with them.
